- Phone: Our phone number is (480) 680-6000. Our goal is to deliver patient-centered experiences, if we are not available, leave a message and we'll be in touch within 2-4 hours. If you are a provider, please contact 480-636-6259.
- Contact Form: If you prefer, you can also fill out our contact form. Just provide your name, email address, and a brief comment of your inquiry. We will respond to your message via email within 24 hours.
Hours of Operation:
- You can reach us Monday - Friday between 8:00AM and 4:00PM during clinic hours. Holiday-hour updates can be found on our google page. We may be closed during certain holidays.
- After-hours: you can reach our designated provider on call for Concierge Members only at 480-636-6259.
- You must dial 9-1-1 immediately in case of a medical emergency.
